Monitoring records <br />include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br />records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs <br />of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. <br />CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br />last three years on site and submit copies to the EHD. <br />This is a minor violation. <br />208 <br />CCR 2716(a -e) Failure to comply with the designated operator inspection requirements. <br />OBSERVATION: The designated operator inspections for the following months were performed more than 30 days <br />after the previous inspection: <br />- Initial inspection on 1/28/2020, next inspection performed on 2/28/2020 (31 days) <br />- Initial inspection on 3/28/2020, next inspection performed on 4/28/2020 (31 days) <br />- Initial inspection on 7/26/2020, next inspection performed on 8/26/2020 (31 days) <br />- Initial inspection on 11/25/2020, next inspection performed on 12/26/2020 (31 days) <br />- Initial inspection on 12/26/2020, next inspection performed on 1/26/2021 (31 days) <br />REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br />the previous inspection. <br />CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br />frames. <br />OBSERVATION: The designated operator inspection for 10/26/2020 was conducted on the outdated form. <br />REGULATION GUIDANCE: An updated UST Designated Operator Visual Inspection Report is to be used effective <br />10/1/2020. <br />CORRECTIVE ACTION: Ensure that all designated operator inspections are documents on the updated form. <br />This is a minor violation. <br />FA0003709 PR0231389 SCO01 01/28/2021 <br />EHD 23-01 Rev. 9/16/2020 Page 5 of 6 Underground Storage Tank Program OIR <br />1868 E.
