OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. -Secondary containment test results from 9/16/20 were not found on site. -Overfill prevention inspection results from 8/27/18 were not found on site. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site. This is a Class II violation. 208 CCR 2716(a -e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The designated operator failed to check and note the correct test dates for the overfill prevention equipment testing on the designated operator inspection reports. Prior to the 9/16/20 inspection, the dates listed include 8/27/18, 1/15/19, and 8/27/19. No record could be found of testing conducted on 8/27/19 or 1/15/19. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the "Facility Employee Training Certificate" on-site. OBSERVATION: The Facility Employee Training Certificate dated 3/16/20 lists incorrect initial training dates and dates of assuming responsibility. REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Ensure the Facility Employee Training Certificate contains accurate information. This is a minor violation.
