QUEST INDUSTRIES LLC,2518 E BOEING WAY, CERS ID: 10184627, PR0538472
Corrective Action Statements
RE: November 04,2020 Hazardous Waste inspection report
Violation 204 -Failed to obtain a permit to treat hazardous waste
❑ Corrected
❑ Violation Corrected by:
0 Supporting Documents Included(analytical results)
Corrective Action:Wash water waste was sampled by Clean Harbors on June 23,2020.Attached analytical
results supported a non-hazardous waste determination. Quest asserts that non-hazardous wastewater was
treated onsite. As a precaution,Quest halted operation of the evaporator as of November 4,2020,to further
investigate the process. If characterization at the point of waste generation results in a hazardous waste
determination, Quest will obtain hazardous waste treatment permit coverage prior to starting operation of
the evaporator unit.
Violation 303 - Failed to maintain all communication or alarm systems, spill control, or
decontamination equipment
❑ Corrected
0 Violation Corrected by: April 15,2021
❑ Supporting Documents Included
Corrective Action: Eye wash station will be removed, repaired or replaced and operation will be tested to
evaluate working condition.
Violation 304-Failed to maintain adequate aisle space for unobstructed response to emergencies
0 Corrected
0 Violation Corrected by: April 15,2021
0 Supporting Documents Included(photos)
Corrective Action: The boneyard area has been cleaned up (photos attached). Materials in the boneyard
identified during the inspection have been moved indoors to improve access to containers for inspection.
The Facility is in the process of implementing changes to the indoor storage location of hazardous waste to
include hazard communication, security measures, aisle space/access, separation of materials, and
secondary containment.Quest anticipates completion of changes by the date indicated above. In the interim,
adequate aisle space to facilitate routine inspection of hazardous waste containers and emergency response
will be provided inside the warehouse.
Violation 403 - Failed to keep hazardous waste containers closed except when adding or removing
hazardous waste
0 Corrected
❑ Violation Corrected by:
0 Supporting Documents Included(photos)
Corrective Action: The 55-gallon drums of dark treated paint waste has been consolidated either with the
liquid paint waste totes or the lined cardboard boxes for paint waste solids. The wastes will be disposed of
as non-RCRA hazardous waste based on current generator knowledge of the materials used onsite. The old
drums have been cut into pieces and added to the solid paint waste containers for disposal. The plastic 5-
gallon bucket of paint waste observed in the compressor room has been consolidated into the liquid paint
waste tote to be disposed of as non-RCRA hazardous waste.
