Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Date: <br />June 25, 2021 <br /> Facility Address: <br /> 351 N BECKMAN RD, LODI <br /> Facility Name: <br /> VAN DE POL ENT INC/PACIFIC PRIDE <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br />OBSERVATION: Designated operator reports were not signed within the required time frame. Multiple designated <br />operator reports were not signed by the owner/operator. <br />REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection, <br />the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 <br />hours of being provided a signed copy of the report, the owner or operator shall provide a description of each <br />corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance <br />issues. <br />CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br />required time frame in the future. <br />=================================================================== <br />OBSERVATION: The designated operator failed to check and note the correct test dates for the Overfill Prevention <br />Equipment testing on multiple designated operator inspection reports. The Overfill Prevention Equipment date <br />written was 4/4/2019 but the correct date was 1/30/2019. <br />REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. <br />CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br />the required information on the reports. <br />This is a repeat violation, Class II. <br /> 308 CCR 2632(c), 2634(d), 2636(f) Failure of the leak detection equipment to have an audible and visual alarm. <br />OBSERVATION: The diesel a/b fill sump sensor failed to come out of alarm when tested to ensure continuous <br />monitoring of the tank system. <br />REGULATION GUIDANCE: All monitoring equipment shall be maintained to activate an audible and visual alarm or <br />stop the flow of product at the dispenser when it detects a leak. <br />CORRECTIVE ACTION: The sensor was replaced and retested during the inspection. <br />This was corrected on site. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 7/25/2021. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />FA0003773 PR0231310 SC001 06/25/2021 <br />EHD 23-01 Rev. 9/16/2020 Underground Storage Tank Program OIRPage 5 of 6 <br />1868 E.
