The Hazardous Materials Inventory forms for Carbon Dioxide were not complete with all the required information. The Hazardous Materials Inventory Form for CO2 was not completed with all the required elements.The federal hazardous categories were not check/fill out. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct the missing information, and submit to the EHD for approval. This is a minor violation. 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. The emergency response(ER)procedures were incomplete.The Emergency Response Plan Section G -number 8 is missing the location description. A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to,the following: -immediate notification of local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment -evacuation plans and procedures, including immediate notice,for the site Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the complete information, and submit to the EHD for approval. This is a minor violation. 9 HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and accurate by the due date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS)annually. Facility did not submit their HMBP for calendar year 2020.The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This was corrected on site on 1/11/2021. This is a Class II violation. Overall Inspection Comments: Conducted in-office Hazardous Materials Business Plans (HMBP) inspection. The food inspector, Stephanie Ramirez, performed an on-site inspection for the CO2 tank on 12/10/2020. Three violations were observed during the in-office inspection. One of the three violation was corrected on January 2021. For the remaining two violations complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 2/13/2021. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
