This information must be current and submitted as part of the <br /> business plan. Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct the missing information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> The emergency response(ER)procedures were incomplete.The Emergency Response Plan Section G -number 8 <br /> is missing the location description. A business plan shall include the following emergency response procedures for <br /> a release or threatened release of hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> complete information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually. Facility did not submit their HMBP for calendar year 2020.The hazardous materials <br /> inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of <br /> the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This was corrected on site on 1/11/2021. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Conducted in-office Hazardous Materials Business Plans (HMBP) inspection. The food inspector, Stephanie <br /> Ramirez, performed an on-site inspection for the CO2 tank on 12/10/2020. <br /> Three violations were observed during the in-office inspection. One of the three violation was corrected on <br /> January 2021. For the remaining two violations complete and submit a copy of the Return to Compliance <br /> Certification form to the EHD with a statement documenting the corrective actions that have been or will be <br /> taken for each violation, and any supporting paperwork, by 2/13/2021. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
