We will need payment before the permit application can go to the inspector to be reviewed. Payment can be made on our website at https://www.sogov.org/department/envhealth/or you can mail us a check. If paying by credit card, please email proof of payment. Attached is a copy of our current UST repair permit requirements and corresponding fee sheet. Please note our email address has changed to ust .sjgov.orq (the paperwork still says ust .sjcehd.com which is incorrect). We also need the attached service request form filled out and submitted to us. We will generate a permit number and assign it to an inspector as soon as we receive payment and the attached service request form. Thank you, 2
