Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site and submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: Designated operator reports were not signed within the required time frame. <br /> -The 4/21/2020 designated operator report was signed by the owner/operator on 9/21/2020 <br /> -The 5/14/2020 designated operator report was signed by the owner/operator on 9/23/2020 <br /> -The 10/2/2020 designated operator report was not signed by the owner/operator. <br /> REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, <br /> the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 <br /> hours of being provided a signed copy of the report,the owner or operator shall provide a description of each <br /> corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance <br /> issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br /> required time frame in the future. <br /> This is a repeat violation, Class II. <br /> 308 CCR 2632(c), 2634(d), 2636(f) Failure of the leak detection equipment to have an audible and visual alarm. <br /> OBSERVATION: The float and chain in the 1/2 UDC and 5/6 UDC failed to stop the flow of product at the dispenser <br /> when tested. <br /> REGULATION GUIDANCE:All monitoring equipment shall be maintained to activate an audible and visual alarm or <br /> stop the flow of product at the dispenser when it detects a leak. <br /> CORRECTIVE ACTION: The service technician adjusted the float and chain and verified functionality. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> FA0006423 PR0231173 SCO01 02/09/2021 <br /> EHD 23-01 Rev.9/16/2020 Page 6 of 7 Underground Storage Tank Program OIR <br /> 1868 E.
