A test report has not been <br /> submitted to the EHD or the California Environmental Reporting System (CERS). <br /> REGULATION GUIDANCE:A copy of the Overfill Prevention Equipment inspection form must be submitted within <br /> 30 days of the test. <br /> CORRECTIVE ACTION: Immediately provide this report to the EHD. <br /> OBSERVATION: Overfill prevention equipment testing was performed on 11/22/19 and a test report was submitted <br /> on 4/24/20,four months late. <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are <br /> performed in a timely manner. <br /> This is a minor violation. <br /> 203 CCR 2712(b)(1-2)Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> -No work orders were found on site, including those for the cold start,the dispenser replacement, and the overfill <br /> flapper replacement. <br /> -Test results from the 5/1/20 overfill prevention inspection were not found on site. <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years.
