SAN J O A R N Environmental Health Department <br /> -:a U ;� i _# �V AMENDED <br /> 4/9/2021 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> STOCKTON MOBIL 3* 1 2358 E WATERLOO RD, STOCKTON April 07, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 214 CCR 2712 Failure to comply with UST operating permit conditions. <br /> OBSERVATION:A cold start was performed on the monitoring panel and a permit was not obtained from the EHD. <br /> The 1/14/21 designated operator report alarm history showed a full history. The 2/11/21 designated operator report <br /> alarm history is mostly blank,with a single sensor having an alarm on 1/29/21. According to Mr. Nasher, a power <br /> outage in January caused the monitoring panel to get stuck in a warm start loop. Mr. Nasher stated a man named <br /> "Douglas"came out to the site on a weekend and fixed the monitoring panel. No work orders were available for this <br /> repair; no notifications were sent to EHD; no certifications were provided for"Douglas". The lack of alarms on the <br /> alarm history printout provided at the time of inspection, dated 4/7/20, indicate that sensors were not tested after the <br /> cold start. <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit <br /> obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work. <br /> CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the cold start of the monitoring panel. A <br /> "Failure to Obtain a Permit"penalty applies. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> finished on April 9, 2021 and replaces the initial checklist. Added violation#207. Removed violation #310. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> NOTE: During fail safe (power out)testing at the very end of the inspection, the 87 relay shut off and could not <br /> be brought back on. It was determined that something had burned in the electrical panel causing it to become <br /> stuck. Elite IV was called to the site at the time of inspection and was able to make the 87 relay functional. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
