COMPLAINT INSPECTION REPORT <br />11:00 am <br />10:25 am <br />Time Out: <br />Time In: <br />Environmental Health Department <br />Name of Facility: BURGER KING #9261 Date: 05/05/2021 <br />Address: 700 N MAIN ST, MANTECA 95367 <br />Owner/Operator: JONATHAN ALIABADI Telephone: (650) 906-1264 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: CO0053743 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT WENT TO FACILITY ON 4/30/21 AT AROUND 3:00 PM. WHEN THE COMPLAINANT RECEIVED THEIR <br />ORDER, THEY NOTICED THEIR FRENCH FRIES HAD BLACK RESIDUE ON THEM COMPLAINANT INFORMED <br />MANAGER AND MANAGER INFORMED COMPLAINANT THAT IS HOW THE FRENCH FRIES COME. COMPLAINANT <br />BELIEVES THE OIL IS DIRTY OR OLD. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br /> #35 Equipment/Utensils Approved and in Good Repair <br />OBSERVATIONS: Walk in freezer has ice build up on the floor. Repair unit in 1 week. As per manager, maintenance <br />personnel is coming in 2 days to service the unit. <br />CALCODE DESCRIPTION: All utensils and equipment shall be fully operative and in good repair. (114175). All utensils and equipment <br />shall be approved, installed properly, and meet applicable standards. (114130, 114130.1, 114130.2, 114130.3, 114130.4, 114130.5, <br />114132, 114133, 114137, 114139, 114153, 114155, 114163, 114165, 114167, 114169, 114172, 114177, 114180, 114182) <br />OVERALL INSPECTION NOTES AND COMMENTS <br />No Temperature Data Collected <br />FOOD ITEM -- LOCATION -- TEMP º F -- COMMENTS <br />NOTES <br />Complaint inspection. <br />As per Store Manager, she is not aware of the complaint. <br />French fries shipment receipt: It comes frozen to be stored in walk in freezer. <br />During preparation, packs are pulled from walk in freezer (25F) to be stored in top load freezer (22F)by the fryers station. <br />I checked the expiration dated of boxes stored in walk in freezer. All Expire 12/2021. <br />Manager picked randomly a pack and open it, I didn't observe any black residues. <br />As per manager, they specify 2 of 3 fryers for frying the french fries.
