The following is an itemized list of underground storage tank violations that have not <br />been addressed for STOP N SHOP as of February 09, 2021. <br />Open violations from August 31, 2020 inspection <br />Violation #208 - Failure to conduct the designated UST operator visual inspection at least once every 30 <br />days. <br />OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br />reports. The incorrect and missing information included the following from the inspection reports: <br />-missing overfill prevention inspection date <br />-missing secondary containment test date <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and shall check all required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required information on the reports. <br />OBSERVATION: Designated operator reports were not signed within the required time frame. The 10/30/2020 and <br />09/02/2020 reports were not signed. <br />REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection, <br />the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 <br />hours of being provided a signed copy of the report, the owner or operator shall provide a description of each <br />corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance <br />issues. <br />CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br />required time frame in the future. <br />OBSERVATION: The designated operator inspections for the following months were not performed 30 days after <br />the previous inspection: 10/01/2018 to 07/2020. There were not records on site of alarm histories or designated <br />operator reports to indicate the tests were conducted for the period specified. <br />REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br />the previous inspection. <br />CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br />frames. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Page 7 of 9 <br />
