Time In: 1:48 pm <br /> r <br /> Time Out: 2:10 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: ROUND TABLE PIZZA Date: 03/25/2021 <br /> Address: 2715 W KETTLEMAN LN, LODI 95240 <br /> Owner/Operator: RTP101 INC Telephone: (209)368-8600 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00053588 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THE EMPLOYEES DO NOT WASH THEIR HANDS OFTEN, THEY PUT BACK ANY <br /> TOPPINGS THAT FALL ON THE PREP TABLE BACK INTO THE CONTAINER TO SERVE AGAIN,THEY DO NOT <br /> CHANGE OLD TOPPINGS, THEY FLIP THE CONTAINER AND PUT NEW TOPPINGS ON TOP. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I went to the facility and spoke to the manager who was not ware of complaint prior to this investigation. <br /> No food prep was occurring during investigation. <br /> Per manager, hands are washed when there is a change of task. <br /> I observed food debris on pizza prep table. <br /> Manager demonstrated how debris is wiped off of pizza prep table. <br /> Garbage Canis placed next to pizza prep table and table is wiped down with wiping cloth and debris is wiped into the trash <br /> can. <br /> Per manager,when prepped items run low they are replaced with fresh items. <br /> I observed prepped containers of olives in walk-in refrigerator. <br /> I observed mushrooms in prep top pizza refrigerator. <br /> Manager turned over mushrooms with a spoon, I did not observe'old toppings" . <br /> Signature not captured due to COVID <br /> FA0017709 C00053588 SCO04 03/25/2021 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 2 Food Program Complaint Inspection Report <br /> 1868 E.
