Time In: 10:56 am <br /> Time Out: 11:20 am <br /> COMPLAINT INSPECTION REPORT <br /> Name of Facility: MANILA EXPRESS Date: 12/21/2020 <br /> Address: 7840 WEST LN, STOCKTON 95210-3338 <br /> Owner/Operator: PHILLIP LAM/SHARON NGUYEN Telephone: (209)478-2896 <br /> Program Element: D004 -2020 COVID-19 Complaint#: C00053134 <br /> Responsible Party: <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THEY ARE NOT WEARING MASKS AND ARE NOT SOCIAL DISTANCING IN THIS FACILITY. <br /> FACILITY IS ALSO FILTHY AND EXTREMELY CROWDED. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> NOTES <br /> I went to the facility and spoke with the owner who is aware of the complaint. <br /> Per owner, he has been receiving several large orders over the phone and the orders are not picked up. <br /> The owner also states that there is a customer that orders food and eats food and then the customer states that they did not <br /> like food and wants their money back (complaint has been by the same customer and has requested for money back multiple <br /> times after eating food, per owner) <br /> During the investigation, I observed pork buns at the counter. <br /> Per owner buns have just been placed on counter. <br /> Time logs are not being maintained. Begin time logs today. <br /> I observed owner and employee at the time of my investigation wearing face coverings. <br /> I observed facility and observed one hand sink that is draining into a bucket. <br /> I observed piece of pipe that is to replace pipe under hand sink that is in disrepair. <br /> Operator to repair today and send proof of correction. <br /> I observed facility and facility was not"Filthy and extremely crowded." at the time of my investigation. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.
