This form should not be altered or modified from how it was released for use by the State. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are not completed on altered reports <br /> forms. <br /> This is a repeat violation, Class II. <br /> 304 CCR 2631(a); HSC 25290.1(c) Failure to construct, operate, and maintain primary containment as product-tight. <br /> OBSERVATION: Fuel was observed coming out of the upper right side piping in dispenser 8 indicating a leak in the <br /> primary containment. Fuel only leaks from dispenser 8 when it is running. <br /> REGULATION GUIDANCE:All primary containment for the UST system must be product tight. <br /> CORRECTIVE ACTION: Immediately have a properly licensed,trained, and certified contractor repair or replace the <br /> failed component(s), under permit and inspection of the EHD if necessary. <br /> This is a Class II violation. <br /> 306 HSC 25291(e)Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the diesel and 87 STP sump. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: The service technician removed approximately 0.5-1 gallon of liquid from the diesel STP <br /> and 87 STP sumps.
