Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COQXZNQTU Site Location: 800 E PESCADERO AVE <br /> Activity Summary <br /> Activity Date Recorded by <br /> 10/01/2020 Received the following email from Brandon Espinoza ALANIZ <br /> <allenvironmentalservicesllc@gmail.com> on 10/1/2020: <br /> "Quick update. I still have not received payment so we have not disposed of the <br /> waste yet." <br /> Called the facility safety manager,Anthony Harb, 209-221-7515, and left a <br /> message for him regarding the waste still being on site. <br /> Received a call back from Anthony and updated him about the waste still being <br /> there. <br /> 10/12/2020 Discussed and emailed the photos I took of the waste generated from the spill FLORIDO <br /> clean up to John Alaniz, REHS. The waste remains on-site at McLane Distribution <br /> Center. Photos taken during McLane's routine hazardous waste inspection on <br /> 9/23/20. <br /> 10/12/2020 Received photos from Elianna Florido, <eflorido@sjgov.org>, who conducted a ALANIZ <br /> site inspection in September. The labels on the hazardous waste containers were <br /> incomplete. Emailed Brandon Espinoza <br /> <allenvironmentalservicesllc@gmail.com> and Sophia Sanchez <br /> <sophia@dtltransport.com> asking for the containers to be labeled. <br /> Received a call back from Brandon, 209-648-8060, letting me know that the waste <br /> was disposed of. He emailed me a copy of the disposal records. Asked Brandon <br /> for a TSDF signed copy of the manfiest and analytical results. <br /> 10/12/2020 Discussed complaint with lead senior Elena Manzo, and Elianna Florido, REHS. ALANIZ <br /> The waste is still on site and needs to be disposed of. The facility the spill <br /> occurred in is a small hazardous waste generator and it was inspected in late <br /> September. <br /> 12/02/2020 Received an email from Presley Sparks, claims specialist, ALANIZ <br /> <sparkp1 @nationwide.com>, asking about the invoice. Called her at <br /> 515-508-3173, and left a message. Sent her an email explaining billing process. <br /> 12/02/2020 Looked up manifest 003623313GBF and found the TSDF signed copy. Called ALANIZ <br /> and emailed Brandon Espinosa, cleanup contractor, at <br /> <allenvironmentalservicesllc@gmail.com> and 209-648-8060. Asked him to send <br /> me the analytical records for the non-hazardous waste. <br /> 12/08/2020 Called Brandon Espinoza <allenvironmentalservicesllc@gmail.com>, ALANIZ <br /> 209-648-8060, and left a message asking for the waste determination documents. <br /> Sent him an email. CC'd the responsible party-Sophia Sanchez <br /> <sophia@dtltransport.com> <br /> 12/10/2020 Received disposal records from Brandon Espinoza ALANIZ <br /> <allenvironmentalservicesllc@gmail.com> on 12/10/2020.
