Loomis Rd. <br /> Activity Summary <br /> Activity Date Recorded by <br /> 04/03/2020 12:48 am GAITAN <br /> COROPPNGK-Received call from Rudy-supervisor for Clean Harbors. Let me <br /> know that Eduardo Macias was the supervisor on site again. Sent text to Eduardo <br /> requesting an update on clean-up. He responded stating that there is one-hour left <br /> of clean-up. <br /> 2:12 am Met with Eduardo Macias, supervisor, Clean Harbors. He opened up <br /> nearby drain and stated that organic peroxide release never impacted drain. <br /> Boons were placed there by Montezuma Fire District as a precaution. Drain <br /> observed to be dry and no signs of being contaminated with organic peroxide. <br /> Eduardo showed me that soil on fence line were never impacted with organic <br /> peroxide release, since there was a curb there preventing that. He states soil were <br /> used by Montezuma Fire to help contain the release. Eduardo explained that they <br /> neutralized organic peroxide release with acid neutralizer. They checked rinsate <br /> with pH strips and kept neutralizing until pH results came back between 5-7 on pH <br /> meter. I requested he do additional test pH test on floor areas that still appeared <br /> wet. He performed pH test on 5 sites, all results came back neutral, pH between <br /> 5-7. We checked shipping truck contaminated with organic peroxide, an odor was <br /> still noticeably coming from the truck. He states truck was neutralized, and the <br /> smell is being aired out. Total waste generated from clean-up: <br /> -Three 55-gallon drums with Waste solids (soil, absorbent) contaminated with <br /> organic peroxide. <br /> -Three 55-gallon drums with waste organic peroxide with water. <br /> -One 55-gallon drum with organic peroxide. From collection bins of leaking organic <br /> peroxide placed by Montezuma Fire. <br /> -One approximately 3-gallon size trash bag with waste solids contaminated with <br /> organic peroxide. <br /> -Three 3x3 foot boxes with waste solids (ppe) contaminated with organic peroxide. <br /> -Two 3x3 foot boxes with wood waste contaminated with organic peroxide. Wood <br /> waste comes from wood pallets holding hazardous materials. <br /> All waste is being managed as hazardous waste and will remain on site until waste <br /> profiles are completed. Once profiled waste will be transferred to Clean Harbors <br /> West Sacramento location. 2550 Del Monte St, West Sacramento, CA 95691. <br /> Asked Harbor Freight employees to place caution tape around waste generated <br /> from clean-up. Left site after caution tape were set around waste. <br /> 04/03/2020 Updated Elena Manzo (Lead Senior REHS) on ER clean-up. GAITAN <br /> 04/03/2020 Typed narrative, corrected spelling errors. GAITAN <br /> 04/06/2020 Printed photo logs. added to ER file. GAITAN <br /> 04/06/2020 Received call from Joe Guzman, Safety Manager for Oak Harbor Freight. Stated GAITAN <br /> that he is currently waiting on waste profiling to be completed. He provided his <br /> email and stated that he will provide copies of waste profiles and hazardous waste <br /> manifest to me. <br /> Sent email to Joe with my contact information. <br /> 04/20/2020 Sent email to Joe Guzman and Tim Baird, requested if the address on file belongs GAITAN <br /> to Oak Harbor Freight Lines Corporate Office.
