SAN 10 A Q U I N Environmental Health Department <br /> I <br /> WASTE DISCHARGE/SPILL RESPONSE <br /> NOTICE TO ABATE <br /> Responsible Party: PGE Date: Nov 21, 2020 <br /> RP Mailing Address: 1108 MURPHYS GRADE ROAD Record ID#: COSZGWLWK <br /> ANGELS CAMP, CA 95222- Program Element: 2546 <br /> Spill Location: 2357 PISA CIR, STOCKTON Release/Spill Response (excluding Joint Team) <br /> NATURE OF WASTE DISCHARGE/WASTE RELEASE: <br /> A vehicle struck a pad mounted transformer causing the release of approximately 30-gallons of transformer oil,spilled oil flowed onto <br /> soil, vegetation, concrete and asphalt. <br /> OBSERVATIONS: <br /> Onsite to verify PGE spill clean up. David Larsen(with PGE),who was onsite, indicated that the spill occurred between 9 <br /> and 10 pm on 11/20/2020. EHD was notified on 11/21/2020 at 8:56 am. David indicated that the transformer pad was <br /> replaced, and the soil beneath the pad was excavated. David collected a transformer oil sample for PCB analysis. Observed <br /> excavated area around the transformer pad, no staining was noted. Street and sidewalk areas were cleaned up using dry <br /> absorbent.Twelve 55-gallon containers or excavated soil and waste absorbent were generated.All generated waste shall <br /> be disposed of under the appropriate waste manifesting procedures HSC 52160(b)(3). Provide PCB results for transformer <br /> oil to the EHD. <br /> VIOLATIONS: <br /> Discharge of any waste, including sewage <br /> (CA Health and Safety Code 5411) <br /> ° Discharge of waste or sewage to water or within 150 ft. of waterway <br /> (SJC Ordinance Code Sections 9-1125.3, 9-1125.4) <br /> Improper disposal of a hazardous waste <br /> (CA Health and Safety Code 25189.5) <br /> Permitting or Maintaining a Nuisance <br /> (SJC Ordinance Code Section 8-5100) <br /> Other Violation <br /> ORRECTIVE ACTIONS/ORDER: <br /> • Investigate and abate contamination under the oversite of the appropriate lead agency. <br /> • Submit copies of hazardous waste manifests/disposal records to the EHD. <br /> Other Correction <br /> EHD 25-01 Rev.9/16/2020 Page 1 of 2 WASTE DISCHARGE/SPILL RESPONSE NOTICE TO ABATE <br /> 1868 E.
