Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COTJ202RZ Site Location: SMITH CANALAT SHIMIZU DR & OCCIDENTALAVE <br /> Activity Summary <br /> Activity Date Recorded by <br /> 09/25/2020 Received complaint from Claudia Muro, EHS, who states at 8:22AM she was told FLORIDO <br /> by complainant that a porta potty was floating in the waterway near Shimizu Dr and <br /> Occidental Ave in Stockton. Looked up location and the waterway is called Smith <br /> Canal, which connects to the SJC River. Called and left a message for <br /> complainant. Received a call back from complainant who states his boss was told <br /> about the porta potty by a resident that lives at 3042 Canal Dr in Stockton; the <br /> complainant nor his boss have seen the porta potty, nor do they have the resident <br /> contact number; the porta potty is a United Services porta potty; he is concerned <br /> that the porta potty may have hazardous chemicals in it; porta potty does not <br /> belong to his company. Complainant states he is unable to follow up with this <br /> complaint and he is unable to notify Cal OES. <br /> Discussed complaint with Claudia Muro, EHS, Haza Saeed, Ld Sr REHS, and <br /> Melissa Nissim, CUPA PC. <br /> Called Fish &Wildlife (916-826-1110) and provided the complaint information to <br /> Margie (dispatch). Margie states she will have a Warden call me back. <br /> Received a call back from Warden Sam Shutz (209-642-0465). Discussed <br /> complaint with Sam, and he states he will discuss it with his boss and call me back <br /> to let me know if Fish &Wildlife will be responding to this complaint. <br /> Called Rio Vista Coast Guard (707-374-2871) and provided the complaint <br /> information to Seaman Mobley and Seaman Combs. Seaman Combs stated she <br /> would have to get more information from the complainant to determine if the Coast <br /> Guard will need to respond. I told Seaman Combs that the complainant did not <br /> have the resident contact information other than their home address. I also told <br /> Seaman Combs that I notified Fish &Wildlife and was waiting for their call back. <br /> Updated Haza Saeed, Ld Sr REHS. Generated complaint. <br /> Arrived at 3042 Canal Dr. in Stockton and met with Bob and Gae Stewart <br /> (209-946-0897), property owners, who allowed me to enter his backyard to see the <br /> portapotty in the Smith Canal. Observed the portapotty about half submerged in <br /> the canal on the south side of the canal. According to Bob, other homeowners in <br /> the neighborhood tied a rope to the portapotty and tied the other end to a tree to <br /> prevent it from floating away. <br /> Drove to the south side of Smith Canal and observed the portapotty was tied to a <br /> tree, and was located across the street from the residence at 2938 Shimizu Dr. <br /> Leakage from the portapotty, or sheen on the water, was not observed. Dead fish <br /> or vegetation was not observed. The portapotty had signage indicating it belonged <br /> to United Site Services (1-800-TOILETS).Took photos. As I drove away from the <br /> site, I noticed three United Site Services portapotties were setup just west of the <br /> intersection of Shimizu Dr and Ryde Ave. One of the three portapotties had a City <br /> of Stockton health tip sign posted. Took photos. Left site to another ER complaint. <br /> Arrived at EHD. Called and spoke to Fish &Wildlife Warden Shutz, who states he <br /> also observed the portapotty in Smith Canal and he called the 1-800 number for <br /> United Site Services and notified them of the portapotty in the canal. According to <br /> Shutz, United Site Services is going to remove the portapotty from the canal. <br /> Shutz stated he will follow up with United to ensure the portapotty is removed. <br /> Called the Rio Vista Coast Guard and spoke to Seaman Rivera, who stated he <br /> was aware of the portapotty incident. Notified Rivera that Warden Shutz will be <br /> following up with United Site Services to ensure they will be removing the <br /> portapotty from the canal. <br /> 5105.rpt Page 2 of 4 <br />
