SAN 10 A Q U I N Environmental Health Department <br /> TY <br /> Time In: <br /> Time Out: <br /> COMPLAINT INSPECTION REPORT <br /> Name of Facility: CENTRAL VALLEY WASTE SERVICES Date: 01/13/2021 <br /> Address: 1320 S WASHINGTON ST, LODI <br /> Owner/Operator: USA WASTE OF CALIFORNIA INC-CENTRAL VALLEY WASTE SVC Telephone: (209)333-5611 <br /> Program Element: 2546-RELEASE/SPILL RESPONSE(EXCLUDING JOINT TEAM) Complaint* COXSWX073 <br /> Responsible Party Contact: HENRY FONTES <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> CAL OES 21-0202: RP states they had a loose hydraulic line causing a spill of less than 2 pints. Has been stopped and <br /> contained with no waterways impacted. Spilled onto asphalt. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> NOTES <br /> The following information was confirmed by Henry Fontes of Central Valley Waste Services(CVWS)via phone: The hydraulic <br /> fluid released due to a loose hydraulic line/fitting on a Waste Management truck. The fitting was repaired on-site by a CVWS <br /> technician.The spill was 10-12 inches wide by 2 feet long, and impacted the street asphalt only.The truck driver applied <br /> absorbent powder to the spill,swept up the used absorbent, and transported the absorbent to the CVWS facility at 1333 E <br /> Turner Rd. Lodi, CA.The used absorbent will be stored at this facility until disposal. <br /> The following information was confirmed by Josh Maloney, Fleet Manager at CVWS, 1333 E Turner Rd, Lodi:The used <br /> absorbent was placed in a 55-gallon hazardous waste drum,which is picked up for disposal by World Oil every 90 days. <br /> Please be advised that the disposal of any waste[HSC 5411]or disposal of any hazardous waste,or the causing thereof, is <br /> prohibited when the disposal is at a facility which is not permitted or authorized for hazardous waste disposal. [HSC <br /> 25189.5(a).]. Please be aware that all hazardous waste shall be disposed of under appropriate hazardous waste manifesting <br /> procedure[HSC 25160(b)(3)].
