0 0 <br /> MICROPIGIVIENTATION INFORMED CONSENT <br /> NAME: DATE OF BIRTH: <br /> ADDRESS: <br /> CITY: STATE: ZIP: <br /> PHONE: EMAIL: <br /> HOW DID YOU HEAR ABOUT BROWS BY CHELSEA? <br /> Cosmetic tattooing, also called semi-permanent makeup or micropigmentation, is the process of implanting <br /> color pigment beneath the surface of the skin by means of a tiny,sterile, disposable needle to produce the <br /> appearance of cosmetic makeup or enhance the natural features of the face. <br /> Immediately following your procedure,the tattooed area will appear to be darker and bolder in color and <br /> more sharply defined. This can last up to a week after the treatment while the initial healing process takes <br /> place. The treated area may feel a bit like a sunburn afterwards but there should be minimal discomfort. <br /> During the initial healing period,there should be minimal scabbing or flaking of the skin. If there is some <br /> scabbing of flaking, do not pick, and allow the scab or dry skin to come off on its own. <br /> The complete healing process takes about 6-8 weeks, at which time the true color of the tattoo is evident. <br /> During this process,your tattoo will soften and lighten. Be patient, even if you think too much color has been <br /> lost, as your skin is still regenerating and naturally exfoliating. Wait at least 6 weeks to see the true color, as it <br /> seems to take on new strength, and it is only after 6-8 weeks that your touch up/adjustment can be done. <br /> I understand that there may be a certain amount of discomfort or pain associated with the procedure and that <br /> other possible adverse side effects may include: minor and temporary bleeding; bruising; redness or other <br /> discoloration and/or swelling. Fading or loss of pigment may occur. Secondary infection in the area of the <br /> procedure is rare if properly cared for, but may occasionally occur. <br /> I specifically acknowledge that I have been advised of the facts and matters set below. I acknowledge that it is <br /> not always possible to determine in advance whether I might have an allergic reaction to any of the pigments, <br /> dyes,topical preparations or processes used in the procedure. I acknowledge that inks are not FDA approved <br /> and health consequences are unknown; and I agree to accept the risk that such reaction is possible. <br />
