The following is an itemized list of hazardous materials violations that have not been <br /> addressed for J&M TRANSMISSION INC as of March 09, 2021. <br /> Open violations from January 14, 2019 inspection <br /> Violation#7-Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not complete. The consolidated emergency response and contingency <br /> plan is missing the local Unified Program Agencies(UPA)phone number on line C3. Please list 209-468-3420 as <br /> the local UPA phone number. Please list all of the locations for boxes marked in section G. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#9-Failed to annually review and certify that the business plan is complete,accurate,and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually. The business plan was submitted late on 2/16/2018.The business has not made a <br /> submittal for 2019. Please make a submittal for 2019 as soon as possible. Submissions made after 1/15/2019 will <br /> be late. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year.
