The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br /> minimum of 90 days after use. A body art facility shall not conduct business without a valid health permit. A person shall not perform body art without a valid <br /> registration and a permitted facility. A body art facility shall notify the LEA in writing within 30 days of resignation, termination of a <br /> practitioner. A new hire or apprentice must be registered the EHD prior to performing body art at the facility. The operator is awaiting sterilization certificates from Ultimate Tattoo Supply for newly <br /> purchased needles which he anticipates will arrive on 12/18/2019. <br /> 2. The operator is awaiting sterilization certificates from Metal Mafia for newly purchased piercing <br /> needles which he anticipates will arrive in one to two days. <br /> 3. The floor in the clean room has been sealed with a product called Glaze & Seal Clear Coat. <br /> 4. The client consent forms, medical questionnaire, and aftercare instructions have the required <br /> information. <br /> 5. Information was provided for Medi-Checks (MDS 200600) integrators that specifies the <br /> integrators as Class V. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
