HSC 119312 - Permits obtained and available <br /> OBSERVATIONS: <br /> Ian Goff, an unpermitted practitioner, is offering tattoo services at this facility. When this was brought to the facility owner's <br /> attention, she mentioned he is a guest spotter and she was unaware of his permit status. Mr. Goff's permit was reactivated on <br /> March 23, 2026. - CORRECTED <br /> CORRECTIVE ACTIONS: <br /> A person shall not perform body art without a valid registration and at permitted facility. A new hire or apprentice must be <br /> registered with the EHD prior to performing body art at the facility. It is the facility owner's responsibility to verify that all <br /> practitioners possess a valid and current body art permit prior to offering body art services within the facility. The facility owner is <br /> to implement a verification process to confirm practitioner compliance prior to allowing guest artists or new practitioners to <br /> operate at the facility. <br /> Correct all violations and provide evidence of correction to the EHD <br /> by April 8, 2026. <br /> Notes: <br /> 1. Inspection report emailed to facility owner, Christina Ramirez, at talltalestattoo(c).Qmail.com. <br /> 2. Per facility owner, the sharps waste has not been disposed of since the last routine inspection, due to the facility not <br /> generating enough sharps waste. Sharps waste shall be disposed when sharps containers are full or once a year(if containers <br /> are not full). Facility owner shall dispose of sharps waste and provide evidence of disposal to the EHD. <br /> 3. Jewelry is purchased from Metal Mafia. Per Mark Escalante, the jewelry is stainless steel; documentation was provided. <br /> 4. Currently, there are 4 tattoo artists and 1 piercer at the facility. <br /> 5. Few of the photo IDs that were attached to Mark Escalante's client records were too dark and difficult to read. Ensure the <br /> photo IDs are clear and eligible. Also, Mr. Escalante was using the old consent forms which do not have all of the Form B <br /> requirements. Ensure the updated forms are being used. <br /> 6. Current permits were not posted. Ensure permits are posted in a conspicuous place when received. <br /> 7. The Infection Prevention and Control Plan (IPCP)was available for review and practitioners were trained on the IPCP. <br /> 8.
