HSC 119315 - Invoices and log kept for disposable, pre-sterilized equipment <br /> OBSERVATIONS: <br /> A sterilization certificate was not provided for Jessie Witt's Cheyenne needle cartridges with the lot#2024-10-082 or for Mark <br /> Escalante's Metal Mafia needles with the lot#241001. <br /> CORRECTIVE ACTIONS: <br /> A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and client, <br /> date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br /> sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br /> minimum of 90 days after use. Provide EHD with sterilization certificates for the instruments mentioned above. <br /> 7. HSC 119314-Sharps containers labeled, used, and disposed of appropriately <br /> OBSERVATIONS: <br /> The sharps container located in Mark Escalante's procedure room was filled above the fill line. <br /> CORRECTIVE ACTIONS: <br /> The sharps waste container shall be within arm's reach and labeled with the word "sharps waste" or with the biohazard symbol <br /> and the word "Biohazard". Sharp waste containers shall be disposed of by a licensed waste hauler or approved mail back <br /> system. Documentation of proper disposal shall be maintained for 3 years. Ensure all sharps containers are replaced and <br /> disposed of appropriately. Provide photographic evidence of the replacement of the sharp container to the EHD. <br /> 8. HSC 119310, 119315 -Jewelry,tattoo and piercing equipment-clean and sterilized <br /> OBSERVATIONS: <br /> Sterilization certificates were not available for the jewelry. <br /> CORRECTIVE ACTIONS: <br /> Jewelry placed in newly pierced skin shall be sterilized prior to piercing or purchased presterilized. Provide EHD with sterilization <br /> certificates for the jewelry. <br /> 9. HSC 119309 - No eating, drinking or smoking -clean clothes <br /> OBSERVATIONS: <br /> Beverage containers (including alcohol beverage containers)were observed in various trash containers around the facility. <br /> CORRECTIVE ACTIONS: <br /> No food, drink, tobacco product, or personal effects are permitted in the procedure area.
