11i <br /> 1 <br /> NuStar Terminal Operations Partnerships LP <br /> 3505 Navy Drive <br /> Mr. Ray Smith <br /> Page Two <br /> includes six aboveground storage tanks (ASTs), two underground storage tanks (USTs) (UST Nos. 4 <br /> and 6), a fuel-loading rack in the central portion of the site, two metal buildings in the southeastern <br /> portion of the site.' The initial release was reported in 1965, the ground water treatment system and <br /> associated piping were installed in June and July of 2003 and the open case was closed in May 2014. <br /> The tank bottom water collection system is planned to be abandoned in place. This assessment was <br /> conducted to evaluate subsurface conditions in the immediate vicinity of the underground storage tank <br /> and associated piping. The tank is a Xerxes double walled fiberglass 6,000-gallon capacity storage <br /> tank installed in 1987. The tank was used for the dewatering of petroleum tank bottoms from the on- <br /> site above ground storage tanks (AST's). The UST does not meet current UST spill containment <br /> regulations. There is no history of any unauthorized releases from this tank. The tank will be triple <br /> rinsed and all contents removed. The tank will subsequently be filled with 10 sack controlled density <br /> sand slurry and permanently closed. Soil samples were collected prior to abandonment and analyzed <br /> to ascertain potential hydrocarbon concentrations prior to tank abandonment. <br /> SCOPE OF ACTIVITIES <br /> HEALTH AND SAFETY PLAN <br /> A site specific Health and Safety Plan detailing known or potential hazards and emergency response <br /> procedures was prepared prior to field operations. All on-site personnel reviewed the plan and a <br /> "tailgate" safety meeting was conducted prior to initiation of field activities. The plan was maintained <br /> on-site throughout the duration of field activities. <br /> i <br /> UNDERGROUND SERVICE ALERT NOTIFICATION <br /> Prior to conducting any subsurface activities,the areas to be investigated was marked and Underground <br /> Service Alert(U.S.A.)was notified to locate potential utility conflicts. The USA ticket number for this <br /> project is X104103083. <br /> DRILLING AND SAMPLING PROCEDURES <br /> A total of 2 geoprobe exploratory borings and 22 hand auger borings were completed. Locations are <br /> shown in Figure 2. The borings were advanced to 5 feet below surface grade (BSG) for the hand auger <br /> locations and to 15 feet drilled depth(14' BSG) for the geoprobe borings. The borings were divided into <br /> two categories depending on the assessment goal associated with each boring.
