Nitrogen fertilizers,ferrous sulfate heptahydrate, and waste oil have not been reported. <br /> • Violation #6,the site map does not show or address the following: <br /> 1. Four maps were submitted; no maps were numbered (Map 1, Map 2, Map 3, Map 4; see attached <br /> guidance). <br /> 2. Loading areas. <br /> 3. Internal roads. <br /> 4. Emergency response equipment (fire extinguishers). <br /> 5. Storm and sewer drains. <br /> 6. Evacuation staging area. <br /> 7. Emergency Shutoffs. <br /> 8. Access and exit points from the street. <br /> 9. The map does not show the entire facility including the structure where the carbon dioxide for beverage <br /> dispensing is located. <br /> 10. The hazardous materials and storage areas for ferrous sulfate heptahydrate, waste oil, and carbon <br /> dioxide. <br /> 11. A legend to identify the all of the codes/symbols used on the maps has not been provided (FE, SD, ESA, <br /> E.S; see attached map guidance). <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br /> and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". <br /> • Violation #7, Emergency Response/Contingency Plan is not accurate (updated template attached). <br /> 1. Section C. the phone number for the Local Unified Program Agency is wrongly identified at 209-953- <br /> 6211 vs. 209-468-3420. <br /> 2. Section K identifies Scott Dickson vs.Jovito Garcia as the Golf Course Superintendent. <br /> 1 <br />
