Environmental Health Department <br />AMENDED <br />10/28/2020 <br />Hazardous Materials Program Inspection Report <br />Date: <br />September 30, 2020 <br /> Facility Address: <br /> 1331 S HAM LN, LODI <br /> Facility Name: <br /> LODI MUNI SERVICE CENTER <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 8 HSC 25505(a)(4) Failed to establish and electronically submit a reasonable or an adequate training program. <br />The training program was insufficient since the training plan uploaded into CERS is for the underground storage <br />tank program. The hazardous materials business plan training must consist of the information below: <br />Training programs for all employees and annual training, including refresher courses, shall include familiarity with <br />the facility's Emergency response plans and procedures in the event of a release or threatened release of a <br />hazardous material. Training shall cover, but not limited to, all of the following: <br />(A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br />program agency. <br />(B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment. <br />(C) Evacuation plans and procedures, including immediate notice, for the business site. <br />These training programs may take into consideration of each employee. <br />Immediately prepare or update the facility's training program, log into the California Environmental Reporting <br />System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for <br />approval. <br />This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br />Documentation showing relevant initial and annual refresher training to appropriate personnel was not available at <br />the time of inspection. <br />The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual <br />refresher training. All employees shall be trained in safety procedures in the event of a release or threatened release <br />of a hazardous material, including, but not limited to, familiarity with the following plans and procedures: <br />(A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br />program agency. <br />(B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment. <br />(C) Evacuation plans and procedures, including immediate notice, for the business site. This training shall be <br />documented electronically or by hard copy and shall be made available for a minimum of three years. <br />Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br />EHD. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection.
