Environmental Health Department <br />AMENDED <br />10/17/2019 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />October 10, 2019 <br /> Facility Address: <br /> 2292 Daniels St, Manteca <br /> Facility Name: <br /> ULTA BEAUTY #1143 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br />OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The containers <br />were: <br />1. An Oxidizer (OX) Hazardous Waste container missing the accumulation start date and contained the following <br />waste (see photos IMG 8569, 8573): <br />- A nearly empty 3.5 fl oz bottle of ISO Option Waving Lotion (IMG 8570). <br />- A nearly empty 0.85 fl oz bottle of ISO Option Equalizing Pre-Wrap (IMG 8571-2). <br />According to Brittany Kelm, General Manager, the wastes listed above were used by the Ulta Salon employees <br />and then placed into the Oxidizer Hazardous Waste container, since the bottles still hold residual from the <br />products once inside the bottles. <br />2. A State Managed / Toxic (ST) Hazardous Waste container marked with a hazardous property of toxic, although <br />the following waste was being stored (see photos IMG 8574-5): <br />- A 1 fl oz bottle of Maybelline New York Fit Me Foundation was labeled with "Caution: Flammable Until Dry. Do <br />Not Use Near Fire, Flame Or Heat" (IMG 8580-2). When Brittany scanned the waste item, the hand-held scanner <br />classified the item as State Managed Waste (IMG 8591). <br />3. An unlabeled container, missing the words "Hazardous Waste," name and address of generator, hazardous <br />properties, physical state, composition (contents), and accumulation start date, contained the following waste (IMG <br />8595-6): <br /> - A flat iron with its electrical cord cut off from the base of the iron (IMG 8594). According to Brittany, waste hair <br />tools, such as flat irons and curling irons, are supposed to be placed into a State Managed / Toxic (ST) Hazardous <br />Waste container. <br />4. A silver Oxidizer (OX) Hazardous Waste container observed in the Ulta Salon and marked with a hazardous <br />property of oxidizers, although the following waste was being stored (IMG 8600-2): <br />- At least twenty almost empty 2-fl oz bottles of Redken SEQ Gloss of various colors (IMG 8603, 8615). When a <br />bottle of Redken SEQ Gloss 09P (SKU #2295848) was scanned, the hand-held scanner indicated that the waste <br />item was a flammable liquid (IMG 8604-6). <br />REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br />words “Hazardous Waste”, name and address of generator, hazardous properties, physical state, composition <br />(contents), accumulation start date <br />CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste.
