SAN JOAQUIN <br />i./ ` <br />Environmental Health Department <br />AMExDFM <br />10/9/2019 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />ULTA BEAUTY #1152 (Lodi) <br />1423 S LOWER SACRAMENTO RD, Lodi <br />October 07, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />stated that the first salon employee would be arriving today at 3pm on the date of the inspection. <br />- A 4.0 oz aerosol can of Living Proof Perfect Hair Day dry shampoo (IMG 8543-4). <br />- A partially filled 128 fl oz container of Barbicide (IMG 8545-6). However, the scanner identified the waste item as a <br />flammable liquid and solid (IMG 8547). <br />- At least two 4 fl oz tube of Matrix Socolor Cult semi-permanent hair color (IMG 8548, 8557). However, the scanner <br />identified the waste item as a state managed waste (IMG 8549). <br />- At least two containers of Redken Color Stain Remover pads (IMG 8550, 8557). However, the scanner identified <br />the waste item as a state managed waste (IMG 8551). <br />Based on the waste items listed above, the hazardous property and the physical state listed on the container's <br />hazardous waste label is inaccurate/incomplete. Also, based on Christine's statement, the container is missing an <br />accumulation start date. <br />REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br />words "Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br />(contents), accumulation start date. <br />CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. Provide a corrective <br />action statement and supporting documentation to the San Joaquin County Environmental Health Department <br />(EHD) within 30 days. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Notes: <br />- The inspection report was amended by adding violation 102, and by recalculating the amount of hazardous <br />waste generated in the last calendar year (2018) to be 2.23 tons. <br />- On the day of inspection, the inspection checklist was reviewed with Christine Novoa, Merchandise Service <br />Coordinator. However, she stated she was not allowed to sign the checklist and only an individual from <br />corporate could sign. No one from corporate was on site at the time of inspection. I emailed the inspection <br />checklist, the Return To Compliance Certification and Worksheet, the Modified Contingency Plan template, the <br />Hazardous Waste Minimum Labeling Requirements guidance, and a web link to the List of Free CUPA Training <br />Classes to Jeremy Hojnicki, Ulta Beauty Compliance Manager (jhojnicki@ulta.com). <br />An inspection checklist was provided to the facility operator on the day of inspection.
