TASK HAZARD ANALYSIS (Ver.2,June2015) Geosyntec HS Procedures referenced herein are available on Geosyntec s H&S SharePoint site and should be consulted,as appropriate,per project-specific needs.This THA prepared per HS-106-Accident Prevention Program,HS-204-Task Hazard Analysis,and meets the requirements for a"Site-Specific Health and Safety Plan"per regulations and Geosyntec HS Procedures referenced herein(see Section B.14.). PART A - SITE SAFETY PLAN A.1. PROJECT/TASK INFORMATION TASK: Soil Vapor Probe Installation and Sampling Project Name: Lincoln Center Project Number/Org: SAC1111/3210 Project Address: 240 Lincoln Center Stockton,CA,95207 Description of Task& Temporary soil vapor probes will be installed around SG-100 to delineate PCE impacts.Probe installation will not Worksite: commence until locations have been cleared by a private utility locator and Underground Services Alert 811. Once the probe locations have been cleared,Geosyntec will oversee Penecore Drilling, Inc.(Penecore)as they advance up to 10 borings using a direct-push technology drill rig.Geosyntec personnel will log all soil core collected from the borings in order to select soil vapor probe depths.Once depths have been selected,Geosyntec personnel will oversee Penecore as they install temporary soil vapor probes at up to two depths per location(between 5 and 20 feet bgs). Samples will be collected from each soil vapor probe no less than two hours after installation for screening using a HAPSITE mobile laboratory.HAPSITE results will be used to select step-out locations for additional borings and temporary vapor probes up to 10 borings.Samples for laboratory analysis will be collected from the furthest step out probes for analysis by EPA TO-15.Soil vapor probe purging and sampling activities will be conducted in accordance with the soil vapor sampling SOP in Appendix B of the Soil Vapor Assessment Work Plan.An estimated 14 soil vapor samples with two duplicates are expected to be submitted to the laboratory.. Work for this task will take place outdoors on Site parking lots.
