No work <br /> order was attached. <br /> -The UDC sensor location question was left blank <br /> -The June 12, 2020 alarm history was not attached <br /> -The DO marked an incorrect answer answer to the question asking if all compliance issues from the previous <br /> inspection were followed up on <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> ---------------------------------------------------- <br /> ---------------------------------------------------- <br /> OBSERVATION: The designated operator failed to visually inspect the 87 STP sump for leaks,water, or debris <br /> during the 1/29/2021 visual inspection. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall visually inspect all spill containers, <br /> fill pipes,fill caps, and UDC sumps, and any tank top sump that had an alarm for which there is no record of a <br /> service visit,for damage, and for the presence of leaks,water, or debris, and check that the sensors are located <br /> properly. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are inspecting all of <br /> the required areas and noting it on the on the reports. <br /> ----------------------------------------- <br /> ----------------------------------------- <br /> OBSERVATION: The designated operator inspections for the following months were performed more than 30 days <br /> after the previous inspection: <br /> - Initial inspection on 7/10/2020, next inspection performed on 8/10/2020, (31 days) <br /> REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br /> the previous inspection. <br /> CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br /> frames. <br /> This is a repeat violation, Class II. <br /> FA0003607 PR0232388 SCO01 04/26/2021 <br /> EHD 23-01 Rev.9/16/2020 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
