SAN J O A a U I N Environmental Health Department <br /> -- <br /> COUNTY AMENDED <br /> 10/17/2019 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ULTA BEAUTY#1143 2292 Daniels St, Manteca October 10, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> OBSERVATION: At the time of inspection, it could not be demonstrated(not necessarily documented)that <br /> employees who handle hazardous waste were properly trained, based on the following observations: <br /> 1.The following waste was found inside a container labeled as Oxidizer(OX)Hazardous Waste and missing the <br /> accumulation start date(see photos IMG 8569, 8573): <br /> -A nearly empty 3.5 fl oz bottle of ISO Option Waving Lotion(IMG 8570). <br /> -A nearly empty 0.85 fl oz bottle of ISO Option Equalizing Pre-Wrap(IMG 8571-2). <br /> According to Brittany Kelm, General Manager,the wastes listed above were used by the Ulta Salon employees <br /> and then placed into the Oxidizer Hazardous Waste container,since the bottles still hold residual from the <br /> products once inside the bottles. <br /> 2.The following waste was found inside an unlabeled container(IMG 8595-6): <br /> -A flat iron with its electrical cord cut from the base of the iron(IMG 8594).According to Brittany,waste hair tools, <br /> such as flat irons and curling irons, are supposed to be placed into a State Managed/Toxic(ST) Hazardous <br /> Waste container. <br /> 3. The waste listed below was found in a silver hazardous waste container observed in the Ulta Salon and labeled <br /> with the words"This trash can is for Salon color tubes only,""Empty Daily,""Hazardous Waste Satellite <br /> Accumulation Area,""Oxidizers: Liquids and Solids,"and with the generator name and address(IMG 8600-2). <br /> -At least twenty almost empty 24 oz bottles of Redken SEQ Gloss of various colors(IMG 8603, 8615).When a <br /> bottle of Redken SEQ Gloss 09P(SKU#2295848)was scanned, the hand-held scanner indicated that the waste <br /> item was a flammable liquid(IMG 8604-6). <br /> According to Brittany, once the Salon employees are done using products,the employees place the used product <br /> containers into the silver oxidizers hazardous waste container, since the containers hold residual from the products <br /> that were once inside the containers and cannot be thrown into the trash. <br /> The silver hazardous waste container also contained at least four used 4-fl oz bottles of Matrix Socolor Cult <br /> semi-permanent hair color, at least three used 2 oz tubes of Redken Cover Fusion low ammonia hair color cream, <br /> and at least three used 2 oz tubes of Redken Chromatics zero ammonia permanent hair color cream(IMG <br /> 8607-8615).According to the Ulta Potentially Hazardous Waste Management poster, found posted on a wall in the <br /> hazardous waste storage area, hydrogen peroxide based products including hair color, lightener and tint, are to be <br /> placed in a Oxidizer(OX) Hazardous Waste container(IMG 8597). <br /> REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with <br /> proper waste handling and emergency procedures, relevant to their responsibilities during normal facility <br /> operations and emergencies. <br /> CORRECTIVE ACTION: Immediately train the employees.
