Lutterman <br />On-site Safety Officer: William Madison <br />Project Safety Officer: Tom J. Lutterman <br />This site safety and health plan (SSP) must be kept on-site and accessible to all on-site <br />employees during all phases of site operations. The SSP must be reviewed and signed by all on- <br />site employees, subcontractors, and visitors prior to entering the site. General safe work <br />practices are identified in the Smith Environmental Technologies Corporation's Injury and <br />Illness Prevention Manual and apply to all Smith Environmental work sites. <br />The provisions set -forth in this SSP shall apply to Smith Environmental employees and any <br />subcontractors working for Smith Environmental at the job site. All personnel working for <br />Smith Environmental at the job site must read this SSP, and sign the attached Compliance <br />Agreement before entering the work area. Field personnel may deviate from the safety <br />provisions set -forth in this SSP, but only to upgrade or increase the safety requirements. Smith <br />Environmental personnel may suspend work if unsafe working conditions are identified or <br />observed, or unauthorized modifications to the safety provisions set -forth in this SSP are made. <br />If changes in site or working conditions require changes in safety procedures, appropriate <br />amendments to this SSP will be provided by the Smith Environmental project manager or <br />supervisor, with the written approval of a Smith Environmental project safety officer or the <br />Smith Environmental Health and Safety Director. <br />PAGE 2 <br />
