I will place <br />these documents in your file for future reference. I have included a copy of the "Cost Pre-Approval Request" form. <br />Please use this form in the future for requesting pre-approval of corrective action costs. <br />With the following provisions, the total cost pre-approved as eligible for reimbursement for completing the <br />September 4, 1997, SMITH Environmental workplan and the addendum approved by the San Joaquin County EHD <br />in their September and October 1997 letters is $25,670; see the table below for a breakdown of costs. The pre- <br />approved amount includes installations of three vapor wells, two monitoring wells, and one air sparge well; vapor <br />extraction pilot tests; air sparge pilot tests; and a CAP preparation.
