The following is an itemized list of hazardous materials violations that have not been <br /> addressed for Michael's New York Style Pizza, Inc as of May 17, 2021. <br /> Open violations from April 12, 2021 inspection <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #8 - Failed to establish and electronically submit a reasonable or an adequate training program. <br /> OBSERVATION The Employee Training Plan has not been annually certified/submitted as a part of the business <br /> plan. The last submittal was completed on 5/17/2017. The Employee Training plan has not been submitted for <br /> 2018, 2019, 2020 and 2021. <br /> REGULATION GUIDANCE: Per H&SC 25508.2, the business owner/operator shall annually review and certify that <br /> the business plan is complete, accurate, up-to-date and in compliance with Section 11022 of Title 42 of the United <br /> States Code. An annual electronic submittal satisfies these requirements. The annual business plan shall be <br /> submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the <br /> previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, review and submit/certify the <br /> curren 'nformation. <br /> This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #9 - Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS) annually by the due date, January 15.
