SAN JDARIN <br /> Environmental Health Department <br /> u i Y <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Trinity Parkway AMPM ARCO#83819 10715 Trinity Pkwy, Stockton April 15, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The following materials were found on site that have not been reported to CERS. <br /> -one poly tank approximately 70 gallons in size containing a presoak mixture was found in the car wash storage <br /> room <br /> -twenty 15 pound propane tanks were found in front of the store that were not reported <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. <br /> The following items were missing from the map: <br /> -propane in front of the store <br /> -2 fire extinguishers located in the fuel canopy <br /> - 1 storm drain along the car wash drive through <br /> - 1 storm drain at the car wash drive through entrance <br /> - 1 storm drain between dispenser 13/14 and the parking spaces in front of the store <br /> - 1 fire extinguisher behind cashers next to the fridge <br /> -loading areas for fuel and carbon dioxide <br /> -emergency shutoffs on the map should be label as what is being shut off <br /> The following items on the map were not observed on site: <br /> -3 storm drains on Trinity Parkway <br /> - 1 west storm drain on Scott Creek Drive <br /> - 1 fire extinguisher on the east of the store entrance <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> FA0023650 PR0543653 SCO01 04/15/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 2 of 4 Hazardous Materials Program OIR <br /> 1868 E.
