AC T27CCR 20650 — Grading of Fill Surfaces — The flowing areas were subject to ponding <br />• The top deck of FU04, FU05 and FU06 <br />• The Old Austin Road Near FU06 <br />• The Old Forward Site long the south side and top deck. <br />• The Old Forward site west slope has a large depression created by gas well construction activities. <br />Grade the top deck and side slopes to remove areas that collect storm water by 03/15/08. This is still an issue. The <br />rain has stopped and additional time can be allowed, but failure to address this issue by the next monthly inspection <br />will result in a violation. <br />2. AC T27CCR 20590 — Personnel Health and Safety — Site users were not wearing their hard hats an safety vests. During <br />the inspection the following was noted: <br />1. BND driver not wearing hard hat <br />2. White and red transfer trucks not wearing vest or hard hat. <br />3. One of the trucks delivering Treated Auto Shredder Waste (TASW) was not wearing vest or hard hat. <br />Continue to emphasize the important of wearing safety gear when ever the site users are out of their vehicles. <br />3. AC T27CCR 20740 — Equipment — The one of the tippers had a loose safety bar. The bar was loose and swinging. <br />Properly repair and maintain all equipment in a safe condition. The tipper was not taken out of service during the <br />time of inspection, assurances were made that it would be repaired by the following day. <br />4. Notes: <br />A. The active working face is located in the FU -06. The cover used was TASW, PGM, soil and tarps. The site was <br />using two working faces. One for MSW and the other for special waste (soil, bottom ash and treated wood). <br />B. During the routine inspection it was noted that the Golden By Products truck came in and dumped. There was large <br />amount of dust released from the load during this process. It is the understanding of the EHD that as part of the <br />facilities air pollution control measures that the load would be sprayed down with water to reduce dust emissions. <br />This was not done. <br />C.
