STATE OF CALIFORNIA 0
California Integrated Waste
CIWMB 03 (Rev. 4/04) Management Board
INSPECTION REPORT ADDENDUM PAGE 2 OF 4
AGENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department
SITE NUMBER: 39 -AA -0015
RiN11:80II_\i!Ii
Forward Inc.
INSPECTION DATE: 03/28/08
COMMENTS:
INSPECTOR: Robert McClellon
RECEIVED BY
OPERATOR
1. AC T27CCR 20650 — Grading of Fill Surfaces — The flowing areas were subject to ponding
• The top deck of FU04, FU05 and FU06
• The Old Austin Road Near FU06
• The Old Forward Site long the south side and top deck.
• The Old Forward site west slope has a large depression created by gas well construction activities.
Grade the top deck and side slopes to remove areas that collect storm water by 03/15/08. This is still an issue. The
rain has stopped and additional time can be allowed, but failure to address this issue by the next monthly inspection
will result in a violation.
2. AC T27CCR 20590 — Personnel Health and Safety — Site users were not wearing their hard hats an safety vests. During
the inspection the following was noted:
1. BND driver not wearing hard hat
2. White and red transfer trucks not wearing vest or hard hat.
3. One of the trucks delivering Treated Auto Shredder Waste (TASW) was not wearing vest or hard hat.
Continue to emphasize the important of wearing safety gear when ever the site users are out of their vehicles.
3. AC T27CCR 20740 — Equipment — The one of the tippers had a loose safety bar. The bar was loose and swinging.
Properly repair and maintain all equipment in a safe condition. The tipper was not taken out of service during the
time of inspection, assurances were made that it would be repaired by the following day.
4. Notes:
A. The active working face is located in the FU -06. The cover used was TASW, PGM, soil and tarps. The site was
using two working faces. One for MSW and the other for special waste (soil, bottom ash and treated wood).
B. During the routine inspection it was noted that the Golden By Products truck came in and dumped. There was large
amount of dust released from the load during this process. It is the understanding of the EHD that as part of the
facilities air pollution control measures that the load would be sprayed down with water to reduce dust emissions.
This was not done.
C. The following gas migration compliance wells were monitored during the routine inspection:
GP 9 (S) 0.2 % CH4 GP 11 (S) 0.0 % CH4 GP 12 (S) 0.0 % CH4
(1) 0.2 % CH4 (1) 0.1 % CH4 (I) 0.0 % CH4
(D) 0.1 % CH4 (D) 0.8% CH4 (D) 2.4 % CH4
INSPECTOR SIGNATURE: <2.9----- DATE:2.$ ®$ _
