STATE OF CALIFORNIA <br />CIWMB 03 (Rev. 4/04) <br />INSPECTION REPORT ADDENDUM <br />California Integrated Waste <br />Management Board <br />PAGE 2 OF <br />AGENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department <br />SITE NUMBER: 39 -AA -0015 INSPECTOR: Robert McClellon <br />SITE NAME: Forward Inc. <br />RECEIVED BY <br />INSPECTION DATE: 05/23/08 OPERATOR <br />COMMENTS: <br />1. AC T27CCR 20870 — Hazardous Waste — Treated Wood Waste (TWW) found in green waste pile. TWW is prohibited <br />from being recycled. Properly screen all green waste and wood waste loads for the presence of TWW. When the <br />TWW is found it needs to be removed and handled properly. <br />2. Notes: <br />A. The facility is due for the five year permit review. The EHD has received the Solid Waste Facility Permit <br />application and supporting documents. The EHD will begin the 5 year permit review for the facility. <br />B. The facility had a lot of onsite litter. There wind in excess of 45 miles per hour the day previos. The facility utilized <br />portable and stationary litter fences to control offsite litter. <br />C. The area where the subsurface landfill fire was located has been graded and there was no evidence of an active fire. <br />The gas extraction system header pipe was monitored for gas temperature and CO. The temperature and CO were <br />reported as normal. <br />D. The active working face is located in the FU -05. The cover used was TASW, PGM, and soil. The site was using two <br />working faces. One for MSW and the other for special waste (soil, bottom ash and treated wood). <br />E. The perimeter gas migration wells were monitored during the routine inspection. The results are as follows: <br />GP 9 (s) 0.0% CH4 GP 11 (s) 0.0% CH4 GP12 (s) 0.0% CH4 <br />(i) 0.0% CH4 (i) 0.0% CH4 (i) 0.0% CH4 <br />(d) 0.0% CH4 (d) 0.0% CH4 (d) 0.5% CH4 <br />F. The soil gas extraction wells were taken off line earlier this month. The gas levels in the compliance points are in <br />compliance. The probe GP 9 (a) (the old compliance point) appear to be increasing. <br />G. The following Violation and Area's of Concern, listed during the inspection of January 29, 2007 have been resolved. <br />1. T27CCR 20580 —Lighting <br />2.
