STATE OF CALIFORNIA California Integrated Waste <br />CIWMB 03 (Rev. 4/04) Management Board <br />INSPECTION REPORT ADDENDUM PAGE 2 OF 3 <br />AGENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department <br />SITE NUMBER: 39 -AA -0015 INSPECTOR: Robert McClellon <br />SITE NAME: Forward Inc. <br />RECEIVED BY <br />INSPECTION DATE: 08/22/08 OPERATOR <br />COMMENTS: <br />1. AC T27CCR 20820 — Drainage and Erosion Control — Drainage on north -slope of old Austin Road has tumble weeds <br />and other debris in the drainages. Make sure that all drainages and down spouts are cleaned and debris removed as <br />part of the winterization plan for the facility. <br />2. AC T27CCR 20686 — Beneficial Reuse — The Processed Green Material (PGM) pile experienced a fire on August 8, <br />2008. The Environmental Health Department was notified by the operator. The PGM needs to be monitored on a <br />daily basis. In case where the PGM temperature reaches 160 degrees Fahrenheit the operator needs to action to <br />lower the temperature. <br />3. Notes: <br />A. The five year permit review is complete. The EHD has approved the JTD amendment #4 and rejected JTD <br />amendment #3 due to complications with the Partial Final Closure Plan (PFCP) for Stage IA. The facility operator <br />needs to prepare a JTD amendment to combine the necessary portions of the PFCP and the information submitted in <br />JTD amendment #3 into one document and resubmit it as a new JTD amendment #3. The facility operator needs to <br />resubmit amendment #3 by November 22, 2008. <br />B. The active working face is located in the FU -05. The cover used was TASW, soil and tarps. The site was using one <br />large working face. One for MSW and the other for special waste (soil, bottom ash and treated wood). <br />C. The perimeter gas migration wells were monitored during the routine inspection. The results are as follows: <br />GP 9 (s) 0.0% CH4 GP I 1 (s) 0.0% CH4 GP12 (s) 0.0% CH4 <br />(i) 0.0% CH4 (i) 0.0% CH4 (i) 0.0% CH4 <br />(d) 0.0% CH4 (d) 0.2% CH4 (d) 0.0% CH4 <br />D. The soil gas extraction wells were taken off line in May 08. The gas levels in the compliance points are in <br />compliance. <br />E.
