• <br />STATE OF CALIFORNIA <br />California Integrated Waste <br />CIWMB 03 (Rev. 4/04) Management Board <br />INSPECTION REPORT ADDENDUM PAGE 3 OF 4 <br />AGENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department <br />SITE NUMBER: 39 -AA -0015 <br />SITE NAME: Forward Inc. <br />INSPECTION DATE: 10/31/08 <br />COMMENTS: <br />INSPECTOR: Robert McClellon <br />RECEIVED BY <br />OPERATOR <br />1. Violation T27CCR 20690 — Alternative Daily Cover (ADC) — The previous working face was covered using tarps. The tarps <br />were not properly weighted. The winds from the passing storm removed the tarps and the site did not have cover. <br />The tarps must be properly weighted in order to withstand anticipated winds. Immediately begin to weight the tarps <br />to ensure that they will stay in place. <br />2. AC PRC 44014 (b) — Terms and Conditions of the Permit — The site had a large amount of Treated Auto Shredder Waste <br />(TASW) exposed during the last storm event. The Solid Waste Facility Permit (SWFP) restricts use of TASW <br />during times of wet weather (see LEA Conditions 17. r.). <br />3 AC T27CCR 20686 — Beneficial Reuse — The green waste pile in the green waste grinding are had the following issues: <br />• Temperature @ 185 degrees F. Maintain green waste temperatures below 170 degrees F. <br />• Treated Wood Waste (TWW) found in pile. Properly remove all TWW prior to grinding. <br />• Oil filters from servicing grinder disposed of in green waste pile. This issue has been referred to <br />the Hazardous Waste Unit for follow up. <br />4 AC T27CCR 21780 — Submittal Closure Plans — The SWFP was recently subject to a 5 year permit review. During the <br />review it was noted that the Partial Final Closure Plan for Stage 1 A included information that needs to be made part <br />of the Preliminary Closure Post Closure Plan (PCPCP) for the facility. The proposed amendment to the PCPCP has <br />not been received by the Environmental Health Department (EHD). Provide by December 23, 2008. <br />5 AC T27CCR 20939 (b) (1) — Sub -surface Landfill Gas Control Systems — The facility has experienced 4 subsurface <br />landfill fires. Three of the fire occurred in this year. The fires are all located along the toe of the slope. The affected <br />area is immediately adjacent to the horizontal collector. It appears that the landfill gas system is over pulling and <br />creating fires to erupt below the surface of the landfill. You are directed to submit a plan to the EHD outlining how <br />the facility operator will prevent additional fires from occurring. Submit the plan by December 1, 2008. <br />6. Notes: <br />A. The active working face is located in the FU -05. The cover used was TASW, PGM, soil and tarps. The site was <br />using one large working face. The MSW and special waste (soil, bottom ash and treated wood) were using the same <br />working face they were accessing it different angles. <br />B. The facility began to place waste in the new cell. <br />C. The perimeter gas migration wells were monitored during the routine inspection.
