AC T27CCR 20939 (b) (1) — Sub -surface Landfill Gas Control Systems — The facility has experienced 5 subsurface <br />landfill fires. Four of the fires occurred in this year. The fires are all located along the toe of the slope. The latest fire <br />occurred a long the western toe of the old Forward Inc portion of the landfill. The fire occurred along the horizontal <br />collector. The facility operator indicated that they would place soil over the area and monitor it. The operator <br />provided a copy of the facility response plan the pertained to sub -surface landfill fires. It is currently under review. It <br />appears that the landfill gas system is over pulling and creating fires that erupt below the surface of the landfill. The <br />facility shall review and evaluate the gas control system to determine what can be done to prevent further instances <br />of sub -surface landfill fires. The review and analysis of the gas extraction system shall be completed by January 30, <br />2009, <br />2. AC T27CCR 20690 — Alternative Daily Cover (ADC) — Some areas in the tipping area had Treated Auto Shredder <br />Waste (TASW) that was open and exposed to the rain. The Solid Waste Facility Permit condition 17 (r) restricts the <br />use of TASW during periods of rain. Make sure that TASW is covered or not used during rain events. <br />3. AC T27CCR 20580 — Lighting — The site was using three light towers in the tipping area in FU -08. The three light <br />towers did not provide adequate lighting. Provide additional lighting to the tipping face in FU -08 <br />4. AC T27CCR 20740 — Equipment — The safety bar on the west tipper is loose. Properly repair and secure safety bar to <br />prevent and injury to users. <br />5. Notes: <br />A. The active working face is located in the FU -08. The cover used was TASW, PGM, soil and tarps. The site was <br />using one large working face. The MSW and special waste (soil, bottom ash and treated wood) were using the same <br />working face they were accessing it different angles. <br />B. The perimeter gas migration wells were not monitored this month due to machine in for service. <br />C. The following Violation and Area's of Concern, listed during the inspection of August 22, 2008 have been resolved. <br />1. T27CCR 20686 — Beneficial Reuse <br />2.
