AC 27CCR20590 - Personnel Health & Safety - The drivers operated the tippers without wearing safety vests or hard hats. One driver was <br />- walking in -the -active face -area -with -safety vest or hard hat. <br />ar Provide -safety equipment to -all -on site personnels. -,- <br />b. Ensure that all drivers wear safety equipment when they get out of their vehicle. <br />2. AC 27CCR20650 - Grading of Fill Surface - Ponding and low spot area were observed on top of FU -05 and FU -06. <br />a. Grade the fill surface area to promote lateral run-off and to prevent ponding. <br />b. According to Ruben Romirez, the areas were to soft to workon at this time. When weather permits he will regrade those _ <br />areas. <br />3. AC 27CCR20820 - Drainage & Erosion Control - Several erosions observed on slope if FU -01. The ditches along the south footing of FU -01 filled <br />with standing water; there, was nomabermaven.eQt-or- rainag-e—, <br />a. Repair all erosions. <br />b. Repair all ditches and ensure the drainage Is sufficient. <br />4. AC 27CCR20860 -Traffic Control An Allied Waste Truck, driven by Abel, was speeding through the heavy traffic near active face. It was slipping <br />and sliding in mud and almost caused an accident. <br />a. Provide adequate traffic control. <br />b. Ensure all drivers take precaution and slow down in heavy traffic area. <br />5. Violation 27CCR20919.5 - Explosive Gas Control - The concentration of explosive gas in GP 9 - Shallow was 9.6% by volume. The concentrations <br />of explosive gas were as following:_ <br />GP 9 S 9.6% GP 11 S .2% GP12 S .1% <br />1 .5% 1 .1%1 .7% <br />D .2% D .5% _ D 2.8% <br />a. Ensure that the concentration of explosive gas at the property boundary is below 5% by volume. <br />b. Follow the direction described in 27CCR20919.5(c). <br />Notes: -- - <br />______ <br />A. Active face was covered'by the tarp s and- soil. � <br />B. Treated wood waste (TW" was observed in the green waste pile. The TWW was removed by Forward staff during the inspection. <br />C. The treated auto shredder waste was listed as disp6s6f for las"fweek and -this week, according to Ruben. <br />
