The following is an itemized list of hazardous materials violations that have not been <br /> addressed for LODI FOOD & LIQUOR* as of November 30 , 2021 . <br /> Open violations from November 25 , 2020 inspection <br /> Violation #6 - Failed to electronically submit a site map with all required content. <br /> OBSERVATION : A site map was not completed or submitted as part of the business plan . <br /> - Internal roads were not indicated on the site map <br /> REGULATION GUIDANCE : A site map shall contain a north orientation , loading areas , internal roads , adjacent <br /> streets , storm and sewer drains , access and exit points , emergency shutoffs , evacuation staging areas , hazardous <br /> material handling and storage areas , and emergency response equipment. If a site map element is not applicable <br /> for your facility then list it on the map and label as " NA" . <br /> CORRECTIVE ACTION : Immediately log into the California Environmental Reporting System (CERS) at <br /> http ://cers . calepa . ca . gov/, upload the correct or updated information , and submit to the EHD for approval <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel . <br /> OBSERVATION : Current hazardous materials training records for all employees were not found on site . <br /> REGULATION GUIDANCE : The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training . All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material , including , but not limited to , familiarity with the following <br /> plans and procedures : <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency . <br /> ( B ) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons , property , or the environment. <br /> ( C ) Evacuation plans and procedures , including immediate notice , for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years . <br /> CORRECTIVE ACTION : Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> i <br /> I <br /> t <br /> Page 1 of 1 <br />
