The following is an itemized list of hazardous waste violations that have not been <br /> addressed for LODI FOOD & LIQUOR* as of November 30 , 2021 . <br /> Open violations from November 25 , 2020 inspection <br /> Violation #111 - Failed to comply with uniform hazardous waste manifest exception requirements . <br /> OBSERVATION : The facility did not have exception reports on site for Uniform Hazardous Waste Manifests <br /> ( UHWM ) for 003699380GBF (9/19/2019) . <br /> REGULATION GUIDANCE : <br /> (a) A generator who does not receive a copy of the manifest with the handwritten signature of the owner or operator <br /> of the designated facility within 35 days of the date the waste was accepted by the initial transporter shall contact the <br /> transporter and/or the owner or operator of the designated facility to determine the status of the hazardous waste . <br /> (c) A hazardous waste generator who does not receive a copy of the manifest with the handwritten signature of the <br /> owner or operator of the facility to which the generator's waste is submitted , within 60 days from the date that the <br /> hazardous waste was accepted by the initial transporter, shall submit to the department a legible copy of the <br /> manifest, with some indication that the generator has not received confirmation of delivery . <br /> (d ) Generators shall submit the information to the department at: DTSC Report Repository , Generator Information <br /> Services Section , P . O . Box 806 , Sacramento , CA 95812-0806 <br /> CORRECTIVE ACTION : Locate a copy of the destination facility signed manifests and send a copy to the San <br /> Joaquin County Environmental Health Department ( EHD) . If the destination facility signed manifests cannot be <br /> found then send to DTSC a copy of each generator signed manifest and a statement that the designated facility <br /> signed copy was not received by the generator. DTSC address is DTSC Report Repository , Generator Information <br /> Services Section , PO Box 806 , Sacramento, CA 95812-0806 . <br /> Provide a corrective action statement to the EHD within 30 days . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #117 - Failed to submit a written response within 30 days of receiving an inspection report. <br /> OBSERVATION : This business did not provide a written response to the inspection report dated 11 /21 /2018 within <br /> 30 days . The report was delivered to the business on 11 /21 /2018 . <br /> REGULATION GUIDANCE : The operator of the site or facility which receives an inspection report shall submit a <br /> written response to the department or the local officer or agency authorized to enforce this chapter pursuant to <br /> subdivision (a) of Section 25180 within 60 days of receipt of the inspection report, or within a shorter time as the <br /> department or the local officer or agency may reasonably require , which shall include a statement documenting <br /> corrective actions taken by the operator or proposing corrective actions which will be taken by the operator, for <br /> purposes of compliance with this chapter, or disputing the existence of the violation . The local agency, the San <br /> Joaquin County Environmental Health Department, requires response within 30 days . <br /> CORRECTIVE ACTION : Provide a corrective action statement and supporting documentation to the San Joaquin <br /> County Environmental Health Department, <br /> Page 2 of 6 <br />
