SAN J 0 A Q I I I Irl Environmental Health Department <br /> CCUNT <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> PG&E: Stockton Service Center T4040 West Ln, Stockton September 24, 2021 <br /> Other Violations <br /> 2010 See below Unlisted General violation ❑V ❑R ❑COS <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The following monthly designated operator inspection report issues were observed: <br /> -The designated operator failed to check and note the correct test dates for the overfill prevention and secondary <br /> containment testing. <br /> -An outdated form was used from 10/2020 through 02/2021. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. The results of the <br /> designated UST operator(s)visual inspection shall be recorded on the"Designated Underground Storage Tank <br /> Operator Visual Inspection Report"located in Appendix XIII (California Code of Regulations Title 23) <br /> CORRECTIVE ACTION: The test dates were corrected on site. Ensure that designated operators performing <br /> inspections at this facility are including all of the required information on the reports and ensure that the inspections <br /> are documented on the most current form. <br /> This is a minor violation. <br /> 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. <br /> OBSERVATION: The Facility Employee Training Certificate is not being used to record the designated operator <br /> employee trainings. The facility had documentation of the designated operator training dated 1/15/21, however, it <br /> was not on the state form. <br /> REGULATION GUIDANCE: On and after October 1, 2018, upon completion of the facility employee training,the <br /> designated UST operator(s)shall provide a signed"Facility Employee Training Certificate,"located in Appendix XII <br /> (California Code of Regulations Title 23),to the owner or operator for facility employees that have received training. <br /> The certificates shall be maintained on-site or off-site at a readily accessible location, if approved by the local <br /> agency, and provided to the local agency upon request. <br /> CORRECTIVE ACTION: Document all designated operator employee trainings on the"Facility Employee Training <br /> Certificate"and provide the EHD with a copy. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 24, 2021. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
