The following is an itemized list of hazardous materials violations that have not been addressed for Dollar General #14353 as of March 16, 2021. Open violations from February 25, 2019 inspection Violation#6-Failed to complete and/or submit a site map with all required content. The site map was not complete as part of the business plan. The site map is missing adjacent street locations, emergency response equipment(fire extinguishers), no storm drain locations, no exits locations, no evacuation staging area, and no utilities shut off(power,water,gas)locations. Please add missing elements to the site map. A site map shall contain a north orientation,loading areas, internal roads,adjacent streets,storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information,and submit to the EHD for approval. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#11 -Failed to provide or document initial and/or refresher training to appropriate personnel. Employee training records were unavailable for review at time of inspection.The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years. Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency response plans and procedures,the inventory of information required by this article,and the site map required by this section shall be readily available to personnel of the business or the unified program facility with responsibilities for emergency response or training pursuant to this section.
