This information was required to be submitted by <br /> 06-27-2020. This information has not been fully received resulting in a non-compliant status for this facility. <br /> REGULATION GUIDANCE:An operator that receives an inspection report shall have 30 days to implement the <br /> corrections specified in the inspection report and comply with permit conditions. The corrective action shall include <br /> all of the recommendations made in the report. The operator shall submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. <br /> CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is <br /> submitted and all corrections implemented within 30 days of receiving an inspection report. <br /> This is a repeat violation, Class Il. <br /> 203 CCR 2712(b)(1-2)Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site including: <br /> -Monitoring System Certification test results for 2020 <br /> -Overfill Prevention Equipment inspection results for 04-04-2019 <br /> -Maintenance records addressing the L1 (diesel turbine sump)fuel alarm on 01-27-2021 <br /> -Maintenance records addressing the L2 (diesel annular)low liquid alarm on 02-04-2021 <br /> -Maintenance records addressing the L2 (diesel annular)sensor out alarm on 02-04-2021 <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site and submit copies of missing alarm records to the EHD. <br /> This is a repeat violation, Class Il. <br /> 205 CCR 2712(i)Failure to have current UST Monitoring Plan readily accessible at the facility. <br /> OBSERVATION:An approved copy of the monitoring plans were not available for inspection. <br /> REGULATION GUIDANCE:A copy of these plans shall be accessible on site at all times. <br /> CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class Il. <br /> FA0003695 PR0231511 SCO01 05/21/2021 <br /> EHD 23-01 Rev.9/16/2020 Page 5 of 7 Underground Storage Tank Program OIR <br /> 1868 E.
