Failure to keep signed copy of manifest from the designated facility due to work <br />order 4695. The nozzle was not left on site in the HazMat drum. All VST nozzles are needed For core credit from the <br />Manufacturer in order to be rebuilt. There is a core credit of approximately $200.00. 1 checked our records and talked to <br />the technician involved and confirmed no nozzle was left In the drum. tt was a clerical error that should not have been on <br />the paperwork. Talking to the technician Involved he said he was going to place the node in the drum but then realized <br />that he need4 to return the nonle to the manufacturer due to the core credit involved. He crossed off the statement from <br />our copies of paperwork but not from the original issued to you at the time of the service call. <br />In response to violation item 302. The 91 annular poll siring broke during testing. Our technician was able to push the 91 <br />sensor into place while on site to do the survey for the 87 master spill conlainer replacement. We will add a new string to <br />the sensor when we are on site to install the new spill container. This requires 2 technicians in order to replace the pull <br />string. <br />in response to violation item 318. The 87 main spill container failed when tested. We have a new spill container on order <br />and will schedule the install and inspections with EHD as soon as we receive the new spill container. <br />I hope this helps clarify things for you and EHD.
