COMPLAINANT ALLEGES THEY OBSERVED AN <br />EMPLOYEE PREPARING FOOD WITH NO GLOVES, MASK, OR HAIRNET. ACEVEDO <br />Upon arrival, I observed the staff on site all wearing face masks properly at this <br />time, as well as hats being worn or women with their hair tied up properly. I spoke <br />with the manager on site who stated having no knowledge of a complaint of this <br />nature being made but invited me to conduct my inspection. I observed all staff <br />wearing gloves at this time and when I asked the manager about any issues about <br />staff wearing gloves, they stated that an individual on site does contain a skin <br />condition which prevents them from wearing gloves constantly but they practice <br />proper hand washing procedures. The manager stated that all staff are advised <br />and trained to follow proper hand washing procedures and gloves are mandatory <br />where necessary. I advised the manager to speak with the staff on site and remind <br />them of the state mandate to wear a mask properly on site at all times. <br />Official inspection report emailed to 010834@qsrwest.com. <br />Official inspection report discussed with Selena Bautisa, signature not obtained at <br />this time. <br />03/31/2021 Provided a copy of the inspection report to the complainant, to the email provided ACEVEDO <br />on the complaint. <br />5105. rpt Page 1 of 1
