Unfortunately, I ACEVEDO <br />received no answer but I was able to leave a voice mail at this time. In the voice <br />mail I explained the reason for me calling and provided my contact information. <br />04/08/2021 Complaint inspection. See official inspection report. <br />Upon arrival, I spoke with the manager on site in regard to the nature of the <br />complaint. The manager stated that no one has complained or has called to state <br />they found any pest in their food. They then presented me the invoices of the pest <br />control company that services the facility. The facility is serviced by Official Pest <br />Prevention once a month, the most recent service was conducted on April 1st, <br />2021. I proceeded to conduct my inspection after receiving approval from the <br />manager, during this time I observed two dead roached on site. Once was <br />observed near the walk-in cooler and the other was observed underneath the <br />3-comartmetn sink. I observed no sticky traps along the walls or corners of the <br />facility, in which the owner stated they place them out at night and remove them <br />during the day. I explained what I observed to the manager on site, in which they <br />had an associate clean up the areas of the dead pest and stated will sanitize when <br />they mop tonight. <br />Official inspection report emailed to bingjunwen@yahoo.com. <br />Official inspection report discussed with Bing Wen, signature not obtained at this <br />time. <br />ACEVEDO <br />5105 rpt Page 1 of 1
